Insurance Information
Our office does not participate with any insurance plans. It is the patient’s responsibility to verify whether their insurance provides out‑of‑network benefits.
Your insurance policy is a contract between you and your insurance company. We are not a party to that contract; however, we will file your claims on your behalf. Please be aware that some and possibly all of the services provided may be non-covered services and not considered usual, customary and reasonable under the terms of your dental and or medical policy.
Accurate and current insurance information must be provided if you wish us to submit claims on your behalf. All account balances remain the patient’s responsibility, regardless of insurance payment. If insurance has not paid within 45 days, the remaining balance will be transferred to the patient’s account.
